Below is a list of forms which you will need as you go about your project.
Consulting Form: This form is for companies/teams interested in utilizing 3D4E's consulting team. Please fill out the form, and we will get back to you as soon as possible to begin the consulting process. To learn more about the services we offer, please email us at 3d4e.usc@gmail.com.
Purchase Request Form: Fill out this form to request to purchase external items that you may need for your project. Fill out the following information and once received, the 3D4E Executive Board will look over your request and either approve or decline the request depending on how reasonable, effective, or necessary the purchase is to the success of your project. This form is for 3D4E MEMBERS ONLY.
Financial Reimbursement Form: Fill out this form to receive reimbursements from 3D4E E-board/VP of Finances. PLEASE DO NOT MAKE A PURCHASE OR FILL THIS OUT UNTIL YOU HAVE SUBMITTED THE PURCHASE REQUEST FORM AND HAVE BEEN APPROVED. Otherwise you will NOT be reimbursed. This form is for 3D4E MEMBERS ONLY.
